Doctor:

You have expressed interest in membership with the Chicago Society for Surgery of the Hand (CSSH). This is a great organization to meet hand surgery colleagues in the Chicago metropolitan area, foster new friendships, and continue your growth in the field of hand surgery.

Please review membership categories and the requirements for membership in our by-laws (Article IV) which are available on the web site.

With approval of your membership application by the Executive Committee, you will be designated a CSSH Prospective member. You will be required to present at the Prospective Members meeting, which is typically held in mid-January each year, to be considered for Active or Associate Active CSSH membership. You will have 2 years from the date of application approval to present a topic.

PowerPoint presentations by Prospective members are typically 10 to 15 minutes in length. Your talk need not be new. If still relevant, you may present a topic/research project from residency training, fellowship training, or current practice. An interesting case, or cases that you feel offer some educational value to hand surgeons are also appropriate.

Annual CSSH dues are $150, subject to change. Payment is due in January of the academic year (July 1- June 30) in which your membership application as a Prospective member is approved.

There is a per person dinner meeting charge of approximately $110 which covers your meal and beverage expenses. This charge is also subject to change. Once your application for membership is submitted and approved for Prospective membership, you will be responsible for personal dinner meeting expenses.

If interested in attending a meeting(s) before submission and approval of a membership application, you will need to be sponsored by an Active, Associate Active, or Emeritus CSSH member. This member will be responsible for your dinner expense.

All CSSH dinner meetings, unless otherwise notified, are at the Willis Tower - Metropolitan Club, 67th Floor; cocktails 6:30pm, dinner and meeting at 7pm.

All meal reservations are required at least 1 week in advance of a meeting. Due to restaurant policies, the charge to a member cannot be voided if the reservation is canceled 7 or less days before the meeting. In the near future, membership payment for all dinners will be required in advance and will be non-refundable. However, if a cancelation request is made more than 7 days before the meeting, the pre-payment will be applied to a future dinner meeting.

Attendees at CSSH meetings include members, sponsored guest hand surgeons, sponsored hand therapists, sponsored hand fellows, sponsored residents in training, and guest lecturers.

Attire is professional: women may wear a skirt or pants suit, while men may wear a blazer or suit jacket, button down shirt, suit pants, a tie and dress shoes.

The CSSH Program Director coordinates participation in the Prospective Members meeting. Slots to speak fill up quickly, so if interested in Society membership, please complete the membership application. If approved for Prospective member status, schedule your talk by contacting the Program Director.

We look forward to seeing you at future meetings,

The CSSH Officers
